What You Will Do

Under the supervision of the CST Team Lead, the Community Supports Team Substance Abuse Professional provides direct, community-based services to members with identified substance use needs that co-occur with mental health diagnoses.

  • Conduct assessments and develop person-centered treatment plans in collaboration with CST partners

  • Support members with symptom monitoring, substance use education, and recovery-focused interventions aimed at reducing the need for higher levels of care

  • Assist individuals in building coping skills, daily living skills, and positive leisure activities to support long-term recovery

  • Participate in crisis planning and ongoing care coordination to ensure continuity across the continuum of care

  • Support authorization renewals to ensure members receive appropriate and uninterrupted services

  • Work with a high level of professional autonomy while collaborating closely with an interdisciplinary treatment team

  • Contribute to the growth of the service area through community engagement and collaboration with behavioral health and community resources

Pathways to Life, Inc. is seeking a dedicated and highly organized Tailored Care Manager (TCM) to join our team. The TCM plays a pivotal role in supporting individuals with complex behavioral health and/or intellectual and developmental disabilities (I/DD) needs by coordinating care across multiple systems.

The ideal candidate is a licensed or credentialed human services professional with a passion for advocacy and whole-person care. This position is perfect for someone who excels in care coordination, relationship-building, and ensuring compliance with NC Medicaid’s Tailored Plan requirements.

Summary of Position Responsibilities

The Tailored Care Manager is responsible for assessing, planning, coordinating, monitoring, and advocating for services and supports to improve health outcomes and overall quality of life for members. This role requires collaboration with clients, families, providers, community partners, and stakeholders to ensure person-centered care that addresses the medical, behavioral, and social determinants of health.

The TCM ensures that all activities comply with NC Medicaid Tailored Care Management Clinical Coverage Policy and other regulatory requirements.

Essential Duties

Assessment & Care Planning

  • Conduct initial and ongoing comprehensive assessments to identify client strengths, needs, and goals.

  • Develop and update person-centered care plans (PCPs) that incorporate physical health, behavioral health, and social support needs.

  • Ensure care plans are completed timely and in compliance with Tailored Care Management standards.

Care Coordination & Service Linkage

  • Facilitate access to services by coordinating with health providers, behavioral health teams, DSS, schools, and other community resources.

  • Address barriers to care, including housing, transportation, and employment needs, through referrals and advocacy.

  • Track member progress and ensure services are implemented as planned.

Member & Family Engagement

  • Build trusting relationships with clients and families, ensuring their voice drives treatment planning and decisions.

  • Provide education about health conditions, treatment options, and available resources.

  • Support families through system navigation and crisis situations.

Crisis Support

  • Participate in crisis response planning and assist with de-escalation when needed.

  • Develop safety plans and coordinate with crisis response teams and emergency services.

Documentation & Compliance

  • Maintain accurate, timely, and complete documentation in electronic health record systems (e.g., Pangaea).

  • Submit required forms, assessments, and reports within designated timelines.

  • Ensure compliance with HIPAA, Tailored Care Management policies, and Medicaid regulations.

Monitoring & Quality Assurance

  • Conduct regular follow-ups to monitor progress toward care plan goals.

  • Participate in internal audits, quality reviews, and accreditation activities.

  • Track and report member outcomes for performance improvement.

Team Collaboration

  • Work closely with other care managers, supervisors, and clinical staff to promote consistent, coordinated services.

  • Attend team meetings, case reviews, and required agency trainings.

Qualifications

  • Education: Bachelor’s degree in Human Services, Social Work, Psychology, Nursing, or related field. Master’s degree preferred.

  • Experience: Minimum of two years of experience providing care coordination or case management to individuals with behavioral health or I/DD needs.

  • Licensure/Certification:

    • Must hold or be eligible for one of the following: LCSW, LCMHC, LMFT, RN, LCAS, or equivalent licensure.

    • Certified Care Manager credentials preferred or must be obtained within state-required timelines.

Program Development & Operations 

Direct and coordinate all aspects of the Personal Care Program, including program structure, policies, procedures, and service delivery workflows

  • Bachelor’s degree in a Human Services or related health field required, with experience working with the population served; candidates with a non–Human Services bachelor’s degree must possess additional qualifying experience as outlined below.

Job Summary

Pathways to Life, Inc. is opening a brand-new Group Home, and we are seeking an experienced Residential Facilities Director to lead the full start-up, licensing, and operational launch. This role will build the program from the ground up and oversee all aspects of residential operations, staffing, compliance, and program quality.

Key Responsibilities

Start-Up & Program Development

  • Lead the start-up, licensing, and opening of a new Group Home.

  • Develop operational systems, policies, workflows, and staffing structures.

Program Oversight

  • Manage day-to-day group home operations once open.

Staff Leadership

Qualifications

Required

  • Bachelor’s degree in Human Services, Social Work, Psychology, Healthcare Management, or related field.

  • 3–5 years of experience in residential or behavioral health settings.

  • 2+ years of supervisory or management experience.

  • Knowledge of NC group home licensing and regulatory requirements.

  • Valid driver’s license; ability to pass background and drug screenings.

Preferred

  • Experience launching a new residential or behavioral health program.

  • Master’s degree in Human Services or related field.

SUPERVISOR’S TITLE: INTAKE DIRECTOR

SUMMARY OF POSITION RESPONSIBILITIES:

The intake coordinator will meet the criteria of a Qualified Professional, who will assist with scheduling and coordinating and completion of all intakes in relation to all services offered by the site.  The intake coordinator will collaborate with all providers involved in the intake process (including physicians) and maintain an active schedule to ensure a smooth transition of services from provider to consumers.   


ESSENTIAL DUTIES: 

  1. Complete all entrance documentation, providing explanation of services and forms to each client and guardian prior to assessment.

  2. Complete all insurance verification and checks eligibility, 

  3. Applies for IPRS funds for clients without insurance, ensuring all documentation is kept and complete in a timely manner.

  4. Check enrollment with MCO and/or create enrollment if needed.  

  5. Verifies insurance copays with BCBS and other sources as requested. Distribute information to appropriate staff.

  6. Assist with completion of 30 day audits, audits should be complete for all clients by the 5th of the following month for the assigned site.

  7. Access to Care maintenance.  Add all referrals/ intakes to the ATC spreadsheet and assure each column is updated and accurate (daily).

  8. Maintain authorization listing in the EMR, adding units and uploading all official authorization letters.

  9. Uploads client documentation into the medical record (EMR) related to the authorization.

  10. Maintain safety and confidentiality within the intake process

  11. Maintain appropriate professional conduct while working with individuals and families and interacting with staff.

  12. Distribute appropriate and accurate information to professional staff, physicians and relevant clinical providers related to consumers.

  13. Maintain a positive and professional relationship with all stakeholders and community networks.

  14. Communicates information according to company policy and procedures as well as the service definition. 

  15. Demonstrates sensitivity to age, developmental and cultural needs of clients.  

  16. Attend stakeholder and community meetings as assigned by the supervisor.

  17. Provide coverage for administrative assistants daily for 1 hour during lunch.

  18. Performs miscellaneous job-related duties as assigned by supervisors, to include but not limited to performing Administrative Assistant duties as needed and requested.

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS: 

  1.   Intake Coordinator will have a minimum of a bachelor’s degree in a related health field with a strong working knowledge of the company policies, mission, and overall objective.  

  2.   Must have a bachelor’s degree in a Human Service field with two years of full time post bachelor experience or a Bachelor's degree in a non-Human Service field with four years of full time experience with the population served.
